    • Change of Income
      If you are reporting a change in income. Please note that any change in income must be reported to your housing specialist in writing within 10 days of change.
    • Direct Deposit form - Client URP
      If you would like to enroll in direct deposit or change the direct deposit information we have on file to receive your Utility Reimbursement payment. Please read the form carefully. You must send the completed form AND a copy of a voided check or something from your bank showing your name, bank name, account number, and routing number.
